Information for Admitted Students
Admitted students must complete the candidate's reply form and submit a $300 non-refundable deposit to guarantee a place in the program.
Once the reply form is submitted, candidates must also complete the following forms:
Medical History and Release form (pdf)--Must be completed by a physician
Housing Information Form (pdf)
Travel Information Form (pdf), i.e., Arrival and Departure Dates
