Undergraduate Advisement and Registration Forms

The following documents will help you as you register for courses:

• Registration Clearance Worksheet
  This worksheet is to be filled out with a proposed schedule each semester and brought to your registration clearance appointment.
  
  
• Degree Requirement Checklist (for students who entered in or after Fall 2009)
  Degree Requirement Checklist (for students who entered Fall 2008 and before)
  It is important that each student tracks his or her own degree progress. This checklist will help you track degree requirements and plan courses for future semesters.
  
  
• Program Declaration Form
  This form is to be used to officially declare your chosen fields of study. Please complete the form and turn it in to your advisor before the start of your senior year.
