Guide for the Local Coordinator can now be found on the CDC website.
Callers/requestors can be directed to the CDC website at
http://www.cdc.gov/  and then instructed to got to the Publications link where they will find it under the listing of publications for the National Center for Chronic Disease Prevention and Health Promotion.  The direct address is:
  http://www.cdc.gov/nccdphp/patch/  

The site also includes the visual aids  package and is organized so that the entire document or just individual sections may be downloaded.
